I failed to test code before deployment. Almost a million people couldn't use my stuff. They were less than happy, but at least I was quick to patch it.
We deal with the possibility of this every single time we release added functionality or general updates! Testing is a big part of the development process, so many things can go wrong.

And users will never fail to surprise you in creative ways of screwing up an otherwise fool-proof system.
I love my users, I really do, but damn if they can't find the most obtuse ways of breaking stuff.
