I remember this happening, it was such an unfortunate accident. I also think the track wasn't properly setup.
It was properly setup, except it was a really difficult course. And there was no protection against the metal beams.
They padded them after this accident.
Well then if there was no protection from being launched into the steel beams, it wasn't properly setup.
I was going to post the video of the crash but decided not to out of respect. It really is hard to watch.
