It had everything to do with her looking to recover her medical costs for 2 surgeries from their homeowners insurance and nothing to do with being narcissistic or vindictive. She undoubtedly filed a claim and was undoubtedly denied as the insurer was betting she would not sue her nephew (in name only). So she was forced to sue. Happens all the time as a normal course of business.

And then the insurer probably refused to settle because they knew at trial they would choose a jury (instead of just trial by judge who would know what was going on) as by law they are not allowed to mention that the defense is being conducted by the insurer and the jury would think she was a vindictive narcissist and deny her claim. Plain and simple the insurer was taking advantage of the naivety of jurors to save money.

And apparently the reporter (and many others) either didn't know shit about court proceedings or chose to sensationalize the title and copy as click bait.

Very common trial strategy by insurers.
