Personally I'm guilty as charged for points 10 thru 7, and 5 thru 2.
I've learned to be better at point 10 (not writing things down) by immediately throwing things in my agenda -- whether it's making time to think about it or plan it, or to actually carry something out. Even if I'm in the middle of a conversation, most people prefer you're on top of something and kinda rude-ish by interrupting the convo to note down something they need you to do, rather than forget it. That way I tend to have a good list of things to do, and can look back and see what I didn't manage to do (like today, when I napped instead!)
And I'll politely disagree with point 6, or maybe agree in a way they don't want me to. I'm terrible at waking up, I'm a night owl, and can't properly function for quite a while after I wake up. I need to either eat a frog a bit later on, when I can be focused enough to, or maybe waking up is my frog to be eaten.
As for point 4, I tend to use my wife as an excuse (sorry!): 'sure, let me have a look with [the wife] if I can do that', alternatively: 'hmm, I might've had something planned with someone at that time, let me check with them [hint: this may or may not actually happen] and get back to you'
