Considering that the AMA doesn't recognize "excited delirium", it's the medical examiners way of not blaming cops for negligence (or worse).
When you suddenly find yourself being tortured to death by people that you know consider themselves above the law you probably do go into a different mental state. Quite likely extreme terror.
