I'm not sure how obscure my suggestion really is in the US. It's tough to say what's obscure in the case of "foreign" films - what's obscure in one country could have been a hit in another.
Anyway, I recommend El Orfanato (The Orphanage) [2007]. It's a great Spanish horror/suspense film, IMHO. As I understand it it was a success in Spanish-language markets, but I rarely see it on streaming services here in the US, and it doesn't come up very often in discussion or lists on US websites. No one I've recommended it to IRL has ever heard of it, and I recommend it a lot.
Note: it was remade by Hollywood with disappointing results - avoid that version.

That is a pretty famous movie(in my university at least). Every 3 out of 10 people have watched it. And I didn't even know about the US version.
